Just like when you visit any health professional, we will need to have you fill out some paperwork prior to seeing you. To make this process simpler for you and your family, we have provided the necessary forms for you right here on the site. Just click, print, and bring your forms in with you to your visit! Please note, medical forms cannot be emailed. However, please don't hesitate to send us an email at jo@tonalitie.com if you have any questions about which forms you need or how they should be filled! This step is absolutely not necessary to complete, we will have paper forms available for you to fill when you come in for your visit.
